He's aware of the pain in his stomach lessening first: not by much, but lessening nonetheless. He's aware of Kim touching his wrist next as she lifts his hand away from the ice to turn it over.
Sylar, by this point, has had enough of being touched. It isn't a conscious thought, nor decision, but it's enough of something for Kim's fingers to be clumsily (and rather feebly) pried off of his wrist.
His own hands haven't moved, and beyond that, he doesn't respond to her.
